Local radio to promote mental health awareness: a public health initiative

Background
Public health strategies have focused largely on physical health. However, there is increasing recognition that raising mental health awareness and tackling stigma is crucial to reduce disease burden. National campaigns have had some success but tackling issues locally is particularly important.
AimsTo assess the public's awareness and perception of the monthly BBC Cornwall mental health phone-in programmes that have run for 8.5 years in Cornwall, UK (population 530 000).
MethodA consultation, review and feedback process involving a multiagency forum of mental and public health professionals, people with lived experience and local National Health Service trust's media team was used to develop a brief questionnaire. This was offered to all attendees at two local pharmacies covering populations of 27 000 over a 2-week period.
ResultsIn total, 14% (95% CI 11.9–16.5) were aware of the radio show, 11% (95% CI 9.0–13.1) have listened and the majority (76%) of those who listened did so more than once. The estimated reach is 70 000 people in the local population, of whom approximately 60 000 listen regularly. The show is highly valued among respondents with modal and median scores of 4 out of 5.
ConclusionsLocal radio is a successful, cost-effective and impactful way to reach a significant proportion of the population and likely to raise awareness, reduce stigma and be well received. The format has been adopted in other regions thus demonstrating easy transferability. It could form an essential part of a public health strategy to improve a population's mental well-being.

Summary
Since 1858, the General Medical Council (GMC), which is a registered charity, has held a statutory function to maintain a list of medical practitioners registered in the UK. There has always been an expectation and duty that doctors maintain their professional competence throughout their careers but until now this assumption has not been monitored, nor enforced, except through processes relating to their fitness to practise. In 2009, all registered medical practitioners were routinely granted a licence to practise. However, since December 2012, when revalidation commenced, this licence has been subject to review every 5 years. Revalidation is important but questions have been raised about its validity. Some of the issues are covered in the previous chapter.
According to the GMC's website:
‘Revalidation is the process by which licensed doctors are required to demonstrate on a regular basis that they are up to date and fit to practise.… Revalidation aims to give extra confidence to patients that their doctor is being regularly checked by their employer and the GMC.’ (http://www.gmcuk. org/doctors/revalidation/9627.asp)
The change in legislation and guidance has created new relationships between doctors, employers, regulators, patients and the public. The full impact, costs and benefits remain to be seen.
History of revalidation
The Merrison report in 1975 introduced the concept of specialist training and certification and altered the composition of the GMC. It also suggested periodic tests of competence, but ruled these to be outside its terms of reference (Merrison, 1975). This task was passed to the Alment Committee, whose terms of reference were: ‘To review the present methods of ensuring the maintenance of standards of continuing competence to practise and of the clinical care of patients, and to make recommendations’ (Alment, 1976). The report, Competence to Practise, published in 1976, concluded that compulsory re-licensure was not indicated as it was too difficult but that professional development should be encouraged for all doctors on a voluntary basis.
Revalidation for the medical profession has therefore been under discussion for decades. Progress has been slow, through a series of government reports, the introduction of clinical governance, clinical audit and annual appraisal, and a series of scandals relating to doctors.

Background
The World Health Organization (WHO) ICD–10 Primary Health Care (PHC) Guidelines for Diagnosis and Management of Mental Disorders (1996) have not been evaluated in a pragmatic randomised controlled trial (RCT).
AimsTo evaluate the effect of local adaptation and dissemination of the guidelines.
MethodPragmatic, pair-matched, cluster RCT involving 30 practices.
ResultsGuideline practices were less sensitive but more specific in identifying morbidity, but these differences were not significant. Guideline patients did not differ from usual-care patients on 12-item General Health Questionnaire scores at 3-month follow-up or in the proportion who were still cases. There were no significant differences in secondary outcomes.
ConclusionsAttempts to influence clinician behaviour through a process of adaptation and extension of guidelines are unlikely to change detection rates or outcomes.

The aftercare of psychiatric patients discharged from hospital is currently under debate among clinicians and in the media. Our study audited the planning of aftercare for detained patients discharged under section 117 of the 1983 Mental Health Act. Our completed audit cycle showed how discharge planning can be improved yet underlines the difficulties encountered in providing for this vulnerable patient group.

Psychotherapy is recognised as an integral part of general psychiatric education at the level of preparation for the membership exam. Guidelines were first published over 20 years ago by the Royal Medico-Psychological Association (RMPA, 1971). In 1986 these guidelines were reviewed and clarified amid some pessimism about how widely the previous recommendations were being met (Royal College of Psychiatrists, 1986). The shortage of consultant psychotherapists and the competing demands of other service and training commitments were suggested as explanations for this.
